With this surgery:
– The tip of the nose can be thinned,
– It can be narrowed,

– The nostrils and the alar base can be reduced, shaped, fixed
– The projection can be lowered/raised.
Which areas are touched in nasal tip surgery?
When we say the nasal cartilage structure, we mean the following structures extending down from the nasal bone:
– Upper lateral cartilages on both sides,
– Alar cartilages: lateral and medial crus, alar intermediate area and alar lateral cartilages (also known as alar corpus),
– And the septum cartilage on which they sit or are connected.
The skin, subcutaneous tissue, soft tissues and muscle structures covering these structures are the areas treated in nasal tip aesthetic operation.
What is the difference between Tipplasty and Classic Rhinoplasty?
Nose tip reshaping, unlike a complete rhinoplasty surgery, is performed without procedures such as rasping or osteotomy to the nasal bone.
For this reason, the surgery time is shorter and there is no swelling or bruising around the eyes or face afterwards.
However, despite this advantage, patient selection is extremely critical in this surgery.
Because the nasal bone cannot be intervened, the patient should not have on the nasal bones:
– A significant hump in the nasal bridge,
– Crookedness, widthness,
– Irregularity,
– Open roof, Saddle nose, etc. deformities.
Similarly,
– A serious septum deviation or
– Breathing problems.
In other words, patients who do not really have a problem that needs to be corrected in the bone structure and septum, and who have a straight, midline, regular nasal bone (including those with a very small amount of hump), should be evaluated by the doctor and their suitability for tip reshaping should be determined according to this analysis.
Tipplasty Approach in Patients with Thick Skin
In patients who will undergo nasal tip aesthetic operation, the shaping of the nasal tip to the desired dimensions ( that is, raising, narrowing or thinning it) is planned after a detailed consultation with the patient in a way that does not disrupt the harmony with the bone structure of the nose.
At this point, the patient’s skin thickness is a very important factor.
In patients with thick skin, the desired aesthetic results may not always be easily achieved.
However, additional methods for thick skin are:
– Some subcutaneous shaving and thinning,
– Placement of definition grafts (cap grafts)

Just like classic rhinoplasty, it can be applied to patients of all ages; there is no specific age range.
It is generally an ideal choice for patients who:
Do not want to have surgery on their nasal bone;
Afraid of bruising and swelling that may occur after surgery;
Believe that the operation to be performed will meet their expectations.
In addition, nasal tip aesthetics can be a suitable solution for people who want to get results with less intervention and are not as invasive as rhinoplasty.
Tipplasty surgery is especially preferred in patients with the following nasal tip deformities:
– Wide,
– Drooping or drooping tip,

This decision should be made by the surgeon who will perform the surgery, taking into account various factors.
The main elements that need to be evaluated are:
– Duration of the planned surgical procedures,
– Psychological state of the patient,
– Pain threshold,
– Comfort of the surgeon’s surgery,
– General condition and preference of the patient.
In other words, the doctor and the patient should evaluate together according to how long the necessary surgical procedures will take and how comfortable the patient will feel during this period. Local anesthesia is suitable for smaller procedures, but general anesthesia is suitable for a large, extensive work. Which anesthesia is appropriate should be decided by talking to each other. Similarly, the patient and the doctor should decide mutually whether open or closed technique will be performed. Both: open and closed can be the techniques to be used. This should again be the preference of the doctor who will perform the surgery. In cases, where abnormalities, deformations are high, various grafting and more jobs will be performed, the open technique will probably provide more convenience for the surgeon. Of course, this surgery can also be performed using a closed technique. It depends entirely on the surgeon’s own experience and the decision of which method gives better results according to the techniques he has used before.
Duration and Technical Details of Tipplasty Surgery
The duration of the surgery varies depending on whether the case is major or minor.
Considering the scope of the procedures to be performed, the surgery usually takes between 1 and 2 hours.
However, depending on the situation encountered during the operation, especially in cases requiring the use of more grafts, this period may be longer.
In aesthetic nose tip correction surgery, the procedures are mostly concentrated on the lateral alar cartilages, namely:
Lateral intermedius and Crus laterale regions.
Therefore, one of the most important issues in surgery is:
- Cephalic resection of the alar cartilage (removal of the excess part),
- Reshaping the alar cartilage with techniques similar to “overlap”
- Dome sutures made with 5.0 PDS suture material.
Recovery Process After Tipplasty Surgery
Since this operation is not a complete rhinoplasty, the nasal bone is not intervened. Therefore, the recovery process is shorter and more comfortable.
Swelling and bruising usually do not occur. Thermal plaster is not used unless deemed necessary.
Generally:
– Bandage is applied with a sterile strip or a plaster called omni strip.
– If the caudal septum has not been treated, silicone nasal splint application is not required.
After surgery:
– The patient can use glasses.
– Patients can return to social life faster.
– No need to stay in the hospital overnight.
– Light sports activities can be started after approximately 3 weeks.
Bandages are usually removed within 7–14 days.
Complete recovery is completed between 6 months and 1 year, as in rhinoplasty.
Aesthetic Results Obtained with Tipplasty
